A combination of random stratified random sampling of 120 primary sampling units (8 counties x 8 residence-size categories) and a random walk at the collection site to select households was used for data collection. The respondent within the household was selected by the software used to collect answers from the respondent from a list of adult household members. An official letter with a request for cooperation was distributed to respondents at the time of the first contact.
